Spitting out live rounds

I have a problem with my '94 Trapper in .45, it spits out the odd live round, especially if it needs a clean and being worked fast. What causes this? What specific part do I need to keep especially clean?

I think this is due to the design of the 94. It was designed to fire long rifle rounds, not short fat pistol rounds. I may be wrong but I've heard of this happening before.

Converting it to a rifle cartridge is the only way to get it to feed reliably. Other than that just keep it clean.
